Cook
Position Summary:
Person in this position will provide all meals to each facility. Essential Job Functions include those listed below. Other duties may be assigned as needed.

Essential Job Functions:
 • Prepare all meals in accordance with approved menus, meeting meal time schedules.
 • Record client/staff meal census.
 • Maintain high (Health Department) standards, cleanliness, sanitation, including the wearing of hair nets and/or caps at all time within food preparation and serving areas.
 • Familiarize self with Kentucky State Health Codes relating to the operation of a “Grade A” Kitchen and will be in compliance at all times.
 • Prepare requisitions for food and commodities as needed.
 • Receive and store properly all deliveries.
 • Assure that equipment is in good operating condition and that temperatures are within proper standards. Be constantly aware of fire and safety hazards. Report repair/maintenance needs to supervisor.
 • Label, date, and store all left-overs in accordance with sanitary food handling requirements.
 • Maintain professional standards at all times with food preparation and handling, as well as maintaining a high level of good housekeeping.
 • Maintain current inventory on all commodities and stored goods.

Essential Qualifications:
 • Experience in cooking for large groups of people.
 • Possess a current Food Handler’s Health Card.

RN
Position Summary:
Person in this position will be responsible for the provision of routine nursing services to Clients.

Essential Job Functions:
 • Act as Lead Nurse on shift
 • Conduct nursing assessments of Clients
 • Initiation and delivery of Client Treatment Plans.
 • Administration of medication as ordered by Physician or ANP.
 • Documentation of all medication utilization, storage, etc.
 • Meet with staff and treatment team on a once weekly basis;
 • Report to Director of Nursing on a weekly basis;
 • Write and submit required reports and/or provide appropriate data as needed.
 • Attend regular professional education classes/workshops to continuously upgrade job skills or promote the image of the agency;
 • Work in collaboration with a Physician, Physician’s Assistant, and ANP to assess the physical needs of the patient and implement a plan of care.
 • Orders, interprets, and records results of clinical tests and reports results to physicians.
 • Respond to any emergency situations that would necessitate intervention;
 • Insure that all required policies and procedures of WestCare are carried out.
 • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.
 • Other relevant duties as assigned.

Essential Qualifications:
 • Valid drivers’ license and proof of insurance;
 • 5 years recent experience in a Medical facility.

Nurse Practicioner
Position Summary:
Person in this position will be responsible for the delivery of program services, quality client care, regulatory and contract compliance and interagency and community relationships. Essential Job Functions include those listed below. Other duties may be assigned as needed.

Essential Job Functions:
 • Responsible for all aspects of treatment for all clients
 • Provide case management and quality assurance reviews on a monthly basis;
 • Provide staff observation and supervision on a regular basis;
 • Provide supervision of Medical Staff
 • Meet with staff and treatment team on a once weekly basis;
 • Report to Director of Nursing on a weekly basis;
 • Assist the Director of Nursing with program design, development, and implementation;
 • Write and submit required reports and/or provide appropriate data for administrative and marketing needs;
 • Attend regular professional education classes/workshops to continuously upgrade job skills or promote the image of the agency;
 • Work in collaboration with Director of Nursing and Physician to assess the physical needs of the patient and implement a plan of care.
 • Orders, interprets, and records results of clinical tests and reports results to physicians.
 • Prescribes medications and makes recommendations for other therapeutic forms of treatment.
 • Be on 24-hour call for emergencies;
 • Respond to any emergency situations that would necessitate intervention;
 • Insure that all required policies and procedures of WestCare are carried out.

Essential Qualifications:

Counselor Tech
Position Summary:
Counselor Technicians work under the supervision of a Counselor and assist with the delivery of individual, group and family counseling services and completion of documentation required for the clinical record including, but not limited to; implementation of the Individual Service Plan (ISP), daily notes and weekly reviews. Counselor Technicians will attend interdisciplinary treatment team meetings and provide treatment throughout the client’s care. Under the direction of a Counselor Level II or III, Counselor Technicians will assist with crisis intervention and de-escalation of situations to ensure safety to all. Counselor Technicians will follow directions of Counselors to insure proper client care. Counselor Tecnicians will implement an approved curriculum as the basis for group and individual counseling.

Essential Job Functions:
 • Under the direction of a Counselor Level I or above, utilize counseling and psychotherapy to evaluate and treat emotional and mental problems and conditions, whether cognitive, behavioral, or affective.
 • Under the direction of a Counselor Level I or above, provide individual, group and family counseling.
 • Under the direction of a Counselor Level I or above, take necessary actions to provide crisis intervention. Implementing WestCare procedures regarding crisis intervention as appropriate.
 • General client supervision in daily living activities and general program operation.
 • Provide direct counseling services to clients.
 • Monitor and assess client behavior and report problems/observations to immediate supervisor.
 • Under the direction of a Counselor Level I or above, implement Individual Treatment/Service Plans.
 • Report to immediate supervisor any unusual incidents that should be brought to the attention of the Program Director, Regional Vice President or regulatory agency.
 • Complete documentation required for clinical records and other reports as assigned.
 • Attend all interdisciplinary treatment team meetings.
 •  Requires regular and consistent attendance to ensure continuity of counseling services to clients.
 • Provide transportation as directed by immediate supervisor.
 • Maintain all data collection logs on a daily basis.
 • Assist with overall facility maintenance and general housekeeping.
 • Follow all WestCare policies and procedures.

Education:
 • Minimum of High School Diploma or GED with strong preference for Associate or Bachelor Degree in social or behavioral services field.

Counselor II
Position Summary:
Counselors are primarily responsible for the delivery of individual, group and family counseling services and completion of documentation required for the clinical record including, but not limited to; development of the Individual Service Plan (ISP), psychosocial assessment, daily notes and weekly reviews. Counselors will attend interdisciplinary treatment team meetings, monitor treatment throughout the client’s care, and establish parameters for discharge planning. Counselors are responsible for crisis intervention and de-escalation of situations to ensure safety to all. This includes assessment of crisis situations to determine severity and making recommendations to direct care staff to insure consistency and appropriateness of staff responses to issues. Counselors may direct and supervise staff assigned to them. Counselors will develop an approved curriculum as the basis for group and individual counseling.

 • Utilize counseling and psychotherapy to evaluate and treat emotional and mental problems and conditions, whether cognitive, behavioral, or affective.
 • Assess need and make decisions regarding the delivery of the appropriate individual, group and family counseling.
 • Conduct interviews and make assessments of potential clients to determine need and appropriateness for program participation and referrals.
 • Analyze situations and take necessary actions to provide crisis intervention. Interpreting and implementing WestCare procedures regarding crisis intervention as appropriate.
 • Supervise and direct the activity of direct care staff.
 • Provide direct counseling services to clients.
 • Monitor and assess client behavior.
 • Create Individual Treatment/Service Plans.
 • Report to immediate supervisor any unusual incidents that should be brought to the attention of the Program Director, Regional Vice President or regulatory agency.
 • Participate in decision to refer clients for ancillary services, i.e., family support groups, continuing care groups, etc.
 • Complete documentation required for clinical records and other reports as assigned.

 • Ensure that all staff are following WestCare polices and procedures.
 • Make recommendations to immediate supervisor regarding performance evaluations of program staff.
 • Make recommendations to immediate supervisor regarding interviewing and hiring of direct care staff.
 • Requires regular and consistent attendance to ensure continuity of counseling services to clients.
 • Follow all WestCare policies and procedures.
